Keep a title, content, category and sources with each record.
Review
Agents propose memories for human review. A proposal is not an approved fact.
Corrections
Correct a record without overwriting its previous version.

Limits
- Capture is explicit
- Memory does not automatically capture every action in an external agent session.
- Search depends on indexing
- Semantic search requires the configured embedding service. The stored record remains available if indexing fails.
- Access follows the record
- Private records require authorization. Organization membership alone does not grant access to them.
